The sole issue for hearing in this matter was which employer, if any, was
responsible for three sets of outstanding medical bills. Two sets of bills
were incurred prior to any of the above referenced employers coming on the
risk.
After formal hearing on his claim, the claimant entered into three Form 15s,
Settlement Agreements, in which he agreed with Ouellette Plumbing and
Heating Corporation and two prior employers that the employers would pay
the medical bills incurred during the claimant's employ- ment with each of
the employers. All of the Form 15s have been approved and accepted by the
Department of Labor and Industry. There is no claim outstanding with
regard to the defendant Commercial Piping during the periods covered by
either of the insurers.
THEREFORE, all claims by Dennis Hennessey for medical bills against the
defendants herein are dismissed with prejudice.
